How do I prepare the quarterly HSC packet?

The trigger

The quarterly health and safety committee meeting is two weeks out and papers are due. The default packet is status updates that invite noting, not deciding. This recipe builds one that leads with decisions and carries the officer due diligence evidence trail behind them.

The inputs

• The control register and verification records for [SITE]: last on-site test, method, result and review triggers per [CONTROL]
• Every AI or work-system [CHANGE] this quarter, with its risk assessment and consultation record
• The officer's evidence file: questions asked, records inspected, decisions, recheck dates
• Worker and HSR concerns since the last meeting
• Last meeting's minutes and open actions

The steps

1. List the decisions the committee is asked to make, one line each: question, accountable owner, deadline. Move status items to an appendix.
2. Extract control verification status from the register. An unverified control is a hope, not a control, so the duty to verify means marking it unverified, never effective.
3. Compile the consultation items. Any [CHANGE] to how work is allocated, paced, measured or monitored is a WHS change requiring consultation; record what consultation ran and what remains open.
4. Assemble the officer due diligence trail: what was asked, inspected and decided, plus recheck dates.
5. Run the prompt below to draft the packet, decisions first, adapting the decision memo skill file: options include doing nothing, evidence only from supplied material, gaps named.
6. Verify every AI-drafted line against the source records, then hand the packet to the chair for sign-off.

met. Flag every gap for the chair.

The checks

• Trace each control status line to its register entry; dates must match the source.
• Every consultation item matches an actual record; where none exists, the packet says so.
• Each decision item includes the do-nothing option and labels its recommendation a draft.
• Any control without an on-site verification record is marked unverified.
• Sweep for personal names or incident identifiers before the draft leaves your hands.

Governance controls

• The committee chair (or responsible officer) reviews and owns the packet; AI drafts and organises, the chair decides and answers for the contents.
• AI-drafted summaries are verified against the source registers and records before inclusion.
• The packet records consultation, it does not satisfy it: consultation is a genuine opportunity for affected workers and their HSRs to influence a decision before it is made.
• De-identify worker and incident material before it enters an AI tool: [WORKER_ROLE] and [SITE], never names.
• Record each item's decision, owner and recheck date, so next quarter's trail starts here.

The output

One committee packet: decisions first, then control verification status, consultation items and the officer evidence trail, with status material appended, filed with the minutes.
